As part of providing a psychological service to you, I (Elise Raymond, Registered Psychologist, operating as Home Base Psychology) need to collect and record personal information that is relevant to your situation. This may include your name, contact details, medical and psychosocial history, and other information necessary for providing psychological services to you. The collection of this information is a necessary part of the psychological assessment and treatment process.

Your personal information is collected as part of your assessment and treatment, is stored securely, and, in the interests of your privacy, is used only by me. This information is retained to document what occurs during sessions and to enable me to provide a relevant, informed, and effective psychological service.
Where information is provided electronically (for example, through online forms or telehealth systems), it will be stored digitally in secure systems.

If you choose not to provide personal information in the manner described in this policy, I may not be able to provide psychological services to you. In most circumstances, it will not be possible for you to remain anonymous or to use a pseudonym.

You are entitled to request access to the personal information I hold about you, subject to exceptions outlined in relevant legislation. I may discuss with you the most appropriate form of access if a request is made.

All personal information collected during the provision of psychological services is kept confidential, except where:
Disclosure is required or authorised by law (for example, by court order or subpoena); or
In my reasonable professional judgement, failure to disclose the information would place you or another person at serious risk to life, health, or safety; or
You have provided prior consent for disclosure, including to:
a) Provide a written report to another professional or agency (such as a GP, school, or lawyer);
b) Discuss relevant information with another person (such as a parent, employer, or healthcare provider);
c) Disclose the information in another agreed way;
d) Share information with another professional or agency where this is directly related to the primary purpose for which the information was collected; or
e) Discuss de-identified case material with a board-approved supervisor to ensure best practice per Psychology Board of Australia requirements.

If unauthorised access, disclosure, or loss of personal information occurs, I will take all reasonable steps to minimise the risk of any consequential serious harm.
